The patient record
One record per patient — history, care plans, clinical notes, documents, consents and balance — isolated per clinic in the database itself, with reads journalled as well as writes.

Reminders and recalls
SMS and email that go out on their own — proved against the consent the patient actually gave, held back during quiet hours, and recorded on the patient's thread.
